In order for DEDP to administer ANY type of medication to your child, we are required by law to have this form on file for your child.

Individual Health Care Plan Form (PDF)
If your child has a medical condition that requires an EpiPen or inhaler, please complete this form in addition to the medication consent form.

EEC Emergency Care Plan (PDF)
This form will give us more information on your child's allergies.
